Chronic Disease Management

Chronic Disease Management Program
The Chronic Disease Management (CDM) program is designed to help prevent and manage certain long-term health conditions.
This free service is available to patients who hold a medical card or GP visit card, and who have been diagnosed with one or more of the following conditions:

Asthma
-
COPD (Chronic Obstructive Pulmonary Disease)

Type 2 Diabetes
-
Cardiovascular Disease (including heart failure, angina, prior heart attack or stroke)

Atrial Fibrillation
As part of the program, you will be invited to attend two check-ups each year, which include blood tests and a review with your GP. These visits allow you and your doctor to:

Monitor your health regularly

Detect and address any issues early

Access support and advice for effective management of your condition
Participation is voluntary, but we strongly encourage patients to take part to help maintain and improve their overall health and well-being.
